The assumption has always been simple: Training = Performance. Healthcare has never been short on training. Every nurse, physician, and other licensed personnel can point to the endless stream of mandatory modules, in-services, and annual check-offs that fill the calendar. If people complete the training, performance will improve — or so we tell ourselves. But history shows otherwise. That’s why we need to talk about methods of validation in healthcare training.
For medical device companies, the assumption is even stronger: once clinicians are trained, the device will be used correctly. That assumption feels safe, but it is rarely tested.
Why validation matters: the three domains
Competence has never been one-dimensional. I cringe when I hear leaders talk about “covering” content or checking off attendance. That’s very superficial. True validation should look at all three domains of competence:
- Cognitive (knowing). Can the learner explain, recall, and apply knowledge?
- Psychomotor (doing). Can they use the device or perform the skill safely and consistently?
- Affective (valuing). Do they see the device or process as worth doing — or will they take shortcuts when no one is watching?
Leaders often assume that if staff understand why something matters, they’ll comply. It feels logical — knowledge drives behavior. But research in healthcare doesn’t bear that out. Knowing why is one factor, not the only factor. Without validation of psychomotor skill and affective buy-in, knowing why is just theory. In reality, people shortcut, adapt, or copy a colleague’s bad habits unless all three domains are addressed.
This is why Donna Wright’s work on competency validation has become a cornerstone in nursing education. She outlined multiple ways to validate competence beyond the old standbys of tests and return demonstrations. Building on her framework, I’ve adapted and expanded the list for real-world medical device and healthcare settings.
The menu: 11 methods of validation in healthcare training
The 11 methods by Donna Wright I’m presenting have stood the test of time because they recognize that competence cannot be validated with a single test or checklist.
What I’ve done here is not to reinvent Wright’s model but to translate and expand it for the realities of medical device training. In my world, the issue isn’t just “Can the nurse demonstrate the skill?” but also, “Are they using the device as designed? Are they valuing its safe use? And can the company defend its training if challenged?” Those are the gaps I’m addressing.
Here are 11 methods you can draw from, grouped into three categories: knowledge-based, skill-based, and performance-based.
Knowledge-based methods of validation in healthcare training
- Case study. Present a realistic patient or device scenario and ask learners to analyze it. Strong for testing application, but requires well-written cases.
- Test/exam. Multiple-choice, short answer, or other structured questions. Efficient for large groups, but limited to cognitive outcomes.
- Presentation. Learners prepare and deliver content to peers. Builds both knowledge and communication skills, but can mask shallow understanding if not critiqued well.
- Discussion or reflection group. Facilitated conversation where learners explain reasoning and share perspectives. Great for affective buy-in, but harder to score consistently.
- Self-assessment. Learners rate their own competence and confidence. Valuable for insight into attitudes, but should never stand alone as proof.
Skill-based methods of validation in healthcare training
- Return demonstration. The classic: learner performs the skill while an observer checks accuracy. Still one of the strongest methods, but can be rushed or treated as a formality.
- Mock event with debrief. Simulate a realistic event (device alarm, patient crisis, system failure), then debrief immediately. Excellent for revealing both technical skills and decision-making under pressure.
- Exemplars. A learner describes a real situation where they used the skill, often in writing. Useful for blending skill and valuing, but easy to confuse with reflection journals or case write-ups. (Tip: I keep a cheat sheet to help leaders tell the difference; watch for that in a future freebie.)
Simply “using” a device doesn’t guarantee correct usage. Staff often transfer habits from the old model, mimic a colleague’s method (even if wrong), or use it like a competitor’s brand they know better. Validation means confirming that the device is used as it was designed. Without that, you might as well be validating the wrong product.
Think of “using” the device like making hollandaise sauce. I can describe the steps: whisk egg yolks, drizzle warm butter, add lemon juice. But doing it without breaking the sauce takes finesse. Talking about it isn’t the same as executing it. Validation separates people who can repeat the words from people who can perform the skill.
Performance-based methods of validation in healthcare training
- Evidence of daily work. Chart reviews, audit trails, or documented use of the device in real cases. Useful for ongoing monitoring, but requires solid data systems.
- Peer review. Colleagues observe or give feedback on performance. Builds a culture of accountability, but needs safeguards against bias and favoritism.
- QI monitors (quality improvement metrics). Use existing quality indicators — device utilization reports, infection rates, time-to-intervention data — as indirect evidence of competence. Strong for system-level monitoring, but may be too broad to capture individual performance.
These methods move validation closest to daily practice. They also overlap with the affective domain — motivation, attitude, values. It’s one thing to know why something matters and another to consistently choose to do it right when no one is looking.

The real problem: too many methods, not enough use
Reading through these 11 methods, you might be thinking: We have plenty of ways to validate. Why aren’t we using them? That’s the point.
In most organizations, validation gets reduced to one or two methods — often a test or a quick return demonstration. Everything else? Rarely touched. That leaves massive gaps. You may be validating knowledge but not skill. Or validating skill but not motivation. Or assuming motivation without evidence.
The truth is, having a menu of methods of validation in healthcare training doesn’t matter unless you use them deliberately. Validation is not one-size-fits-all. A return demonstration might be best for a new device, while QI monitors may track ongoing competence across a unit. The challenge is matching the method to the domain and the risk at hand.
The defensibility question
This is where leaders in healthcare and medical device companies should pause. If a regulator, accreditor, or attorney asked how you validate staff competence, would you be comfortable with your answer?
Saying, “We had a training” isn’t defensible. Saying, “We had them take a test” isn’t much better if you can’t show that they can also perform the skill safely and consistently. True defensibility comes from using multiple methods of validation in healthcare training across all three domains, and being able to show proof.
Training ownership and accountability
Here’s the kicker: You can’t pick one of those 11 different methods of validation out of a grab bag and assume it will work. Matching the right method to the right domain and risk takes judgment. I know this stuff, and I still pause to consider which approach will actually reveal competence.
So the real question isn’t whether methods exist. It’s whether you are owning the responsibility to use them.
- Does your training program clearly spell out how competence is validated rather than just assumed?
- Who is accountable for monitoring whether validation happens?
- If you had to describe your validation program today, which word fits: robust, partial, patchy, or nonexistent?
Eventually, every organization has to face those questions. And the sooner you ask them, the safer, stronger, and more defensible your workforce and your product will be.
